All solid points, especially about fiction writers clearly not reading much fiction. I have to disagree about not bothering with other platforms like Simily though. I write everything for Medium because it's the best place to be now, but will still drop some of those stories on Vocal and Simily because it takes less than a minute. If at the end of the year I make $40 from Vocal and $20 from Simily, it covers my Medium membership for essentially no extra work.
